Universal facility agreement.
I certify that my dog(s) is/are in good health, have not been ill with any communicable diseases or parasites in the last 30 days, and have not harmed or shown aggressive or threatening behavior towards any person or any other dog.
I also have read and understand and agree to the following:
Inherent Risks of Play, Training, Boarding & Grooming.
I understand that Partners Dog Training is an open-play environment and because of this there are inherent risks, which even when closely monitored, may result in the following:
Transfer of communicable parasites or an illness such as, but not limited to, the canine papilloma virus also known as "puppy warts," or an upper respiratory illness like Canine Cough, which can be caused by a contagious bacteria or virus.
Injuries, usually benign, such as broken nails, sore pads, puncture wounds, abrasions, and cuts, particularly in shorter coated breeds, etc.
Behavioral problems related to barking, potty, reactivity, etc.
Release
Partners Dog Training and their team will not be liable for any health or behavioral problems that develop in my dog(s), and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at Partners Dog Training.
I am solely responsible for any harm, including to any other dog(s), to the employees or invitees of Partners Dog Training, or to the equipment, physical plant, or other property of Partners Dog Training, caused by my dog(s) while my dog(s) is/are attending Partners Dog Training.
Crate Usage
I authorize my dog(s) to be placed in a crate during the evaluation process, boarding, and/or daycare.
E-Collar/Bark Collar Usage
I authorize my dog(s) to have an e-collar or bark collar placed on them during the evaluation process, boarding, and/or daycare.

Training-program agreement.
Legal Owner: agreement between Partners Animal Institute, Inc. (DBA Partners Dog Training) the Company and the Client whose name and signature appears below. The Client must be the Dogs' Legal Owner and may be required to provide Proof of Ownership. Authorized Representatives need a written letter. The Client must still sign. Only the Client or Legal Representative may pick up, visit, or train a Client dog. No exceptions.
Safety: you are responsible for injuries to others and/or damages from your dog. Company not responsible for dogs that injure themselves training, destroying kennels or biting equipment. Physical abuse or disruptive behavior results in dismissal with no refund. Dogs are not interacted with overnight; supervision may occur from staff members or security systems.
Cancellations: notify the office at least two business days prior or you will be billed. Cancelling/Rescheduling within two business days forfeits the deposit. All reservations require a non-refundable deposit. All fees payable by the time of the reservation start. Prior to start you may credit payment to another service; no refunds or credits after start. Packages are non-refundable; unused portion credited to another service.
Guarantees: dog training addresses and modifies existing behaviors. Modification takes time and effort; results are not all guaranteed.
Photographic Release: the Company may capture, record, and use photos, videos, or media of the Client and their dog(s) for training, promotional, social media, or commercial purposes. Permission is irrevocable; Client waives rights to royalties or compensation.
Termination Of Training: Client may stop at any time. Company may terminate for failure to follow guidelines, differences in philosophies, or personality clashes; will refer to another trainer.
Training Durations: durations are an estimate; actual periods determined by the dog. Private lessons 60 to 90 minutes; group classes 45 minutes.
Deposits and Rates: all payments non-refundable. Camp reservations required a non-refundable $100 deposit; typically half the estimate collected at check-in, balance due at check-out; all balances paid in full prior to check out and release. Class and lesson fees payable at time of reservation. Packages non-refundable. Chargebacks or Bounced Payments incur a $500 fee and payment in cash.
Indemnity: Client indemnifies and holds harmless Partners Animal Institute, Inc., Leighton Oosthuisen, all officers, employees and insurers from any liability. Disputes settled per the American Arbitration Association rules; prevailing party awarded arbitration costs and reasonable attorneys' fees.

Training / pet-parenting philosophy agreement.
What is a Partnership: collaboration is key; we are Partners with you; you and your dog are Partners.
The Foundation: creates structure; structure provides clarity and consistency; Balanced-Based Training identifies acceptable vs unacceptable behaviors; Foundation leads to a well-mannered, balanced dog.
Our Responsibilities (the Company): establish a strong Foundation; teach you to implement it at home; provide tools and strategies; guide you to understand your dog; offer continuous support for maintaining and evaluating progress.
Client Responsibilities: follow the training program; attend all scheduled sessions and follow-up classes; commit to consistent daily practice; apply the methods and suggestions.
Success and Commitment: follow diligently; engage in Pet Parenting classes; attend all demo sessions and practice; monitor progress through check-ins.
Avoiding Failure: failure to follow may lead to regression, decreased results, wasted time and money.
Pet Parenting Transition and Follow-Through: Pet Parenting classes are essential; we teach techniques to apply at home; progress evaluated through follow-ups.
Five Steps to Success: commit to the full process; follow trainer guidelines; attend all sessions and demos; practice daily at home; attend Pet Parenting follow-up classes.

Facility-stay health agreement.
Safety Protocols: committed to safety and health; staff are trained kennel technicians, not medically qualified. Protocols: stainless steel dishes sanitized daily; training areas/kennels/crates disinfected daily; uneaten food disposed after each meal; paper plates for feeding hygiene; dogs monitored during working hours, no overnight supervision; all dogs must be on flea-and-tick prevention (ticks, bees, scorpions risks still exist).
Training and Medical Protocols:
Socialization: controlled play; Client responsible for injuries caused by their dog(s).
Escape Artists: Client must disclose escaping or destructive history.
Aggression History: Client must fully disclose any history of aggression; held responsible for injuries.
Vaccinations: all dogs must have up-to-date vaccination records on file; Bordetella completed at least ten (10) days before check-in.
Medical History: disclose all medical issues from the past six (6) months, incl. exposure to parvo, canine cough, rabies, distemper.
Medication: Company can administer as needed; medications in original container with written instructions.
Special Diets: accommodated; a $40 service charge applies for special order.
Potential Health Risks: Bordetella (Canine Cough), Valley Fever, Canine Influenza. Factors: pre-existing conditions, stress/anxiety/poor eating, compromised immune systems, recent vaccinations.
Client Responsibility for Medical Expenses: Client responsible for medical expenses if dog(s) become ill or injured. Company not responsible for rashes, allergies, happy-tail, nail/tooth damage, ear infections. Client responsible for damages caused by their dog(s) incl. kennels, fencing, crates.
What If Your Dog Requires Medical Attention: Company contacts a trusted vet, attempts to reach the Client, treatment not withheld if health at risk; may send dog home or keep under observation; $50 fee per trip to the vet.
Granting Medical Authority: by signing, Client grants full access to medical data, permission to seek veterinary care and administer medications, and agrees to pay all related veterinary costs during the stay.

Facility-stay communicable-disease agreement.
What is Canine Cough: a respiratory infection from many bacteria and viruses, airborne, spread via coughing/sneezing/surfaces; dogs shed before symptoms; exacerbated by stress, lack of sleep, pollen, humidity/temperature changes.
How we manage it: PDT deep cleans daily with medical-grade disinfectant and a pressure washing system; custom air extraction circulates air over ten times each day.
What Happens If My Dog Contracts It: usually not dangerous, clears in a few weeks; puppies/elderly/existing-condition dogs more susceptible; incubation two to 14 days, five-to-seven-day contagious period.
Prevention: Canine Cough vaccine (Bordetella) required in advance for all dogs that stay at PDT; inoculates against the most common strain; ten known strains exist so risk remains in any boarding/daycare/training/grooming facility.
Liability Release:
Client acknowledges the unavoidable risk of contracting Canine Cough at PDT.
PDT not liable for short/long-term effects due to contracting it.
Client responsible for all reasonable medical expenses incl. prescribed medication; no guarantees as to treatment effect.
PDT continues precautionary measures (cleaning, vaccination record upkeep).
No guarantee of training package extensions or discounts due to contraction.
No refund under any circumstances due to contraction.
Parties: Partners Animal Institute, Inc. (DBA Partners Dog Training), an Arizona corporation (the Company), and the undersigned Member.
Membership Programs and Credits:
Membership Tiers: various options (e.g. Partners Plus, Partners Premium, Partners Platinum) that include a specific number of credits for DaySchool or DayCamp services. Credits defined by the program chosen and the number of visits per month. Each tier has an associated discount on additional offerings.
Credits Usage: non-transferable between member accounts but usable for any dog associated with the Member's account; must be used within the specified time frame; only for DaySchool or DayCamp designated in the tier; not exchangeable for cash or other services.
Membership Term Length: Annual Membership, at least twelve (12) months. By signing, Member agrees to the 12-month term.
Billing, Renewal, and Credit Expiration:
Renewal and Billing Date: renews monthly; payment due on the first (1st) day of each month (Billing Date). Initial renewal date is the first day of the first month the membership was paid.
Credit Issuance and Expiration: each month on the Billing Date a new set of credits is added; each month's credits have a 180-day expiration.
Membership Discounts: active members eligible for discounts by tier; must be active and paid in good standing.
Scope: discounts apply to standard rates for individual reservations, services, or add-ons (additional DaySchool/DayCamp days, boarding nights, Pet Parenting classes, baths, treadmill sessions, etc.).
Exclusions: do not apply to packages or promotional offers unless stated; Camp discounts may be limited over summer and major holidays.
Payments: credits billed monthly on the Billing Date, may appear as multiple charges; once started a membership cannot be paused. Late Payments: not received within ten (10) days is late; Company may suspend access until paid. Chargebacks: Member agrees not to request a chargeback.
Termination and Cancellation:
After the initial 12 monthly renewals, membership renews monthly unless the Member gives written notice of cancellation at least thirty (30) days before the next billing date; notice can be sent as early as 330 days after start.
Early Termination Fee by Member: before the 12-month term, Member pays 25% of the remaining term's membership fees. After 12 months, terminate without penalty via cancellation notice.
Termination by Company: at any time, with or without cause, via verbal or written notice; no early termination fee.
Alternative Termination: in case of Death or Severe Injury by the Member or the Member's dog(s), the Member may terminate with no penalty.
General Provisions: Governing Law Arizona; Entire Agreement; Amendments in writing signed by both; Severability.
